The Future Leaders Awards (“Awards”) takes place between April 1, 2020 and September 17, 2020. The Awards starts with a nomination process in which individuals or corporations will nominate a future leader for consideration as part of the program. Aging Media editors will evaluate each entry in its respective category to determine the Future Leaders class of 2020.
Nominees must be employed by a non-profit, for-profit or governmental agency in the industries Aging Media and its publications covers and/or the businesses that support them. Each award entry fee must be paid prior to the entry deadline in order to be considered for eligibility for consideration and judging. Entry fees are non-refundable. All nominees must be between the ages of 18 - 40.
Nomination Process
Individuals, a corporation or governmental may nominate someone for the awards program via the online submission starting April 1, 2020 and complete the process prior to 5 PM Central Standard Time on June 30, 2020 for consideration. Upon submission, each individual will be entered into the Awards. The form shall include the nominees name, organization, title, email address and information on their work for recognition based upon the program criteria. Each nominee, if not self-nominated, accepts these terms and conditions upon accepting the nomination hereby agrees to the terms and conditions of the Awards. By entering the Awards, entrants and nominees agree to consent to AMN using the personal information for the purposes of evaluating, judging and promoting as part of agreement to participation in the program. Failure to claim or accept the nomination or meeting the eligibility criteria may result in disqualification or forfeiture of the participation in the Future Leaders class.
Competition Details
The Future Leaders Awards application process requires all the following components and responses as part of the submission:
The nominee’s name, title, company, and contact information

The nominator’s name, title, company, and contact information

Confirmation and acceptance of the nominee, if not self-nominated

Current resume or CV (Word or PDF format)

One headshot with appropriate photo credit (PNG or JPG format)

Written Statement: Please provide a 500-word statement outlining the nominee’s qualifications for the program. (Word or PDF format)

Two professional letters of recommendation describing why the candidate excels in leadership based upon the competition’s criteria, with contact information for each advocate.

    Entry Fees:
  • Early bird entries cost $400 USD through May 31, 2020
  • Entries submitted from June 1 to June 30, 2020 are $500 USD

  • Payment method is credit card only. Failure to pay the entry fee will prohibit consideration for the application to the Future Leaders Awards program.
All submissions will be evaluated by the Aging Media editorial staff. The editorial staff may select up to 15 members from the submissions at their sole discretion. The entries selected for the Future Leaders class for each category will be announced online on August 3, 2020 at 12 PM Central Standard Time.
Cancelation or Termination of Program
Aging Media reserves the right to cancel, suspend and/or modify the Awards, or any part of it, if any fraud, technical failures, virus, human error, tampering or any other factor impairs the integrity or proper functioning of the Awards, as determined by AMN in its sole discretion. In such event, AMN reserves the right to award the prizes from among the eligible entries or plays received up to the time of the impairment. AMN reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the or to be acting in violation of the Awards terms and conditions of this or any other promotion or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Awards may be a violation of criminal and civil law, and, should such an attempt be made, AMN reserves the right to seek damages from any such person to the fullest extent permitted by law. AMN failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
Limitation of Liability
AMN is not responsible for interrupted or unavailable network server or other connections; for errors in communication protocols; failed telephone, computer or electronic transmissions; for jumbled, scrambled or misdirected entries or transmissions; for phone, electrical, network, technology or computer hardware or software or program malfunctions, failures or difficulties; or any other errors, omissions, interruptions, or deletions of any kind, whether human, mechanical or electronic; or for non-receipt of any entry regardless of the cause of it not being properly received. AMN is not responsible for unintelligible, late, lost or stolen entries; for incorrect or inaccurate entry information, whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Awards; or for any technical, printing or human errors which may occur in the receipt, non-receipt or processing of any entries in the Awards. AMN MAKES NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE. SOME JURISDICTIONS MAY NOT ALLOW LIMITATIONS OF EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR EXCLUSION OF IMPLIED WARRANTIES. Privacy Policy
Any personally identifiable information collected during an applicant’s participation in the Awards will be collected by AMN and used by AMN in accordance with AMN’s Privacy Policy as such is stated at https://homehealthcarenews.com/privacy-policy/. AMN will share the nominators’ name with the nominee but not with the judges nor with the general public. Beyond the usage described in these terms and conditions, the information provided will be collected, stored and used in accordance with AMN’s privacy policy
Publicity Rights
By submitting nomination, nominees (or persons entering on behalf of nominee) shall allow AMN and/or AMN designees the perpetual right to use his/her entry, name, biographical information, photos, videos and/or likeness, and statements for promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media now known or hereafter discovered, worldwide, including but not limited to on the World Wide Web and Internet, without notice, review or approval and without additional compensation except where prohibited by law. A signed affidavit of eligibility, publicity release and release of liability may be required.
Governing Law
Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of Illinois.
By participating, each nominee releases and agrees to indemnify and hold harmless AMN & Others from and against any and all costs, claims, damages, or any other injury, whether due to negligence or otherwise, to person(s) or property (including, without limitation, death or violation of any personal rights, such as violation of right of publicity/privacy, libel, or slander), due in whole or in part, directly or indirectly, to participation in the Awards.
AMN reserves the sole right to amend or alter the terms and conditions of the Awards at anytime. By entering the Awards, nominee agrees to abide and be bound by these Official Rules and the decisions of AMN, which shall be final in all matters pertaining to this Awards program. All entries become the property of AMN and will not be returned. Void outside of the fifty (50) United States/District of Columbia and wherever prohibited or restricted by law. All federal, state and local laws and regulations apply.
